How to Apply:

1. Download and print application. (You will need Adobe Acrobat Reader to open the file PDF file.)
2. Fill out the application as completely as possible.
3. Ask two of your teachers to send us a one-page recommendation letter.
4. Ask your school to send us a copy of your official grade transcripts. (Unofficial transcripts are not acceptable.)
5. Mail or fax your completed application with responses to questions 1 and 2 to:
